I started Valley Diabetes & Obesity with a mission to treat, educate & empower the underserved community here in Central Valley to live a life to their fullest by maintaining health & wellness. I focus on whole-person care, creating a personalized care plan for each of my patients.I specialize in:- Insulin Pumps and CGMs for patients with Type 1 Diabetes- Insulin therapy for patients on Hemodialysis or Peritoneal Dialysis for patients with Type 2 Diabetes. - Uncontrolled Gestational Diabetes, uncontrolled pre-gestational diabetes and insulin therapy during pregnancy towards prevention of neonatal complications.- Metabolic syndrome, High Cholesterol and Hypoglycemia.-Physician supervised weight loss management combining medications, diet & nutrition counseling, exercise coaching and behavioral therapy.Here are some dire statistics of the Diabetes epidemic that are my motivations:* 23.8% (7.2 million) of people with diabetes are undiagnosed* 34% of US Adults aged 18 years and older with pre-diabetes (as of 2015) and 90% of them don't know that they have it* 1.5 million Americans are diagnosed with diabetes each year* $322 billion - Total cost of care for diagnosed diabetes

- Setup a new Diabetes specialty clinic focused on education, clinical algorithm development and protocols and improving outcomes for patients with diabetes for the underserved and the medicaid community.-Collaborate with community-based resources to facilitate inpatient and outpatient individual and group self-management education options for patients- Provide specialized treatment for T1DM, T2DM, Metabolic syndrome, Gestational Diabetes, High Cholesterol and Hypoglycemia for the low-income and indigent community in the county.-Provide learning opportunities for medical students and residents on preventative diabetes care and advanced diabetes management, including insulin management-Mentor and supervise NPs and RNs in diabetes clinical care protocols to be able to provide individual follow-up care and group self-management.

Currently collaborating with Stanford's ECHO Diabetes initiative to provide best-practice diabetes care and management for the underserved communities in the Central Valley. Clinical Management of patients at the Lone Star Circle of Care Satellite clinics in Central Texas for management of gestational diabetes, pre-gestational diabetes, Family medicine issues and uncontrolled adult and pediatric diabetics

* Clinical Management of patients at the GVHC Satellite clinics in Modesto and surrounding Central Valley, for management of gestational diabetes, pre-gestational diabetes and uncontrolled adult and pediatric diabetics.* Worked with Leadership in setting up guidelines and protocols, identifying and hiring the support staff for the Diabetes clinic. Organized and delivered Diabetes-focused educational talks for the providers.* Provided Diabetes management guidance to local hospital CDE and DNE for the clinic patients who were admitted to the hospital on an ongoing basis.

Delivering expert and specialized diabetes management to patients with new onset diabetes for both Type-1 and Type-2 with chronic medical conditions, pediatric, obstetric and post-partum patients in both outpatient and inpatient settings.

• Delivered extensive medical care and Diabetes care in Adult medicine, Pediatric, Obstetrics, Gynecology, Geriatric, Musculo-skeletal including basic Psychiatric services.• Responsible for a panel of more than 2,000 patients as their Primary Doctor and was involved in primarily out-patient care & follow-up, all minor office procedures and referrals.

Reviewed guidelines, set goals for physicians and coordinated the care with nursing managers. Closely monitored patient education, diet/exercise, medication plans, labs and continuity of care & improvement in patient’s condition.* Cholesterol Champion (‘03-’05) – Represented the Modesto Physician Team and worked with the other Cholesterol teams in Central Valley and Northern California to discuss and present new strategies to improve cholesterol management for patients. Reviewed guidelines, set goals for physicians and coordinated the care with nursing managers. Closely monitored patient education, diet/exercise, medication plans, labs and continuity of care & improvement in patient’s condition.* Member of Pharmacy Committee (‘03-’04) – Worked with committee members bi-weekly to review and update medication list in a variety of patient care settings to better serve the patients.* Member of QA Committee (‘03-’07) – Engaged with other Committee members monthly to review patient care, discuss and plan strategies for improving patient care in all possible medical areas.
