The Complex Neuro Navigator post holder provides a strategic overview, supporting clients' with neurological disabilities as they move from acute services through to in-patient services and onto community services. The Complex Neuro Navigator has an understanding of pathways accessed by all clients with a neurological diagnosis who access rehabilitation services, this will include clients with a deteriorating condition and single neurological events. It also has a role in networking and liaising with neurological services.The Complex Neuro Navigator valuates the local specialist rehabilitation service provision and take a strategic lead on the design and implementation of a collaborative care pathway for people with complex neurological disability, from admission to the acute sector to discharge back into the community who may be placed out of borough or who are referred for out of borough intervention.It will also provide expert clinical opinion on the appropriateness of referrals for level 1, 2a, 2b provision and out of borough assessment, inpatient rehabilitation and works in partnership with other health and social care providers, promoting an integrated approach to the management of progressive and non-progressive neurological conditions.
