Head of Clinical Services for a GP Federation in North East Hampshire. Responsible and accountable for two clinical teams providing community based services in a variety of settings to reduce unplanned hospital admissions. Leading and line managing a multidisciplinary team. Also responsible for developing new clinical services.

- Line management of Specialist and Advanced Paramedics including performance management, yearly appraisals with reviews, facilitation of clinical and non-clinical training subject to identified training needs. Responsible for cascade training and disseminating clinical, operational and organisational updates.- Implemented changes after reviewing existing working practices to improve availability of home visits to member practices including instigating cross-locality working, improving productivity from staff and streamlining processes making referrals to the service quicker and more reliable.- Led and championed service transformation by identifying changes in service demands and identifying opportunities for service development including introducing new services to benefit the health needs of the local population, creating and presenting business cases, implementing these in practice, reviewing post-implementation and leading a cycle of continuous service improvement.- Established an MSc academic pathway with a local HEI to enable recruitment of Student PPs.- Implemented and championed Independent Prescribing for PPs including policy creation and subsequent audit of individuals' prescribing incorporating reviews of the prescribing scope of practice.- Managing and building relationships with multiple external stakeholders regarding service including CCG Medicines Management Team, Primary Care Commissioning Team, HEI, Local Training Hub, PCN Clinical Directors, Ambulance Service Trusts.- Established a NEHF Paramedic in Primary Care Network to improve communication, mutual learning and support especially for Paramedics working in practices as the only Paramedic.- Implemented and championed the HEE Roadmap for Paramedics within the PP Team and wider NEHF Paramedic Network. This included creating a clinical supervision framework and directly supervising Paramedics with monthly COTs and CBDs.- Providing clinical and operational advice to PPs, GPs and AHPs.

- Advanced Paramedic Practitioner in a Rapid Home Visiting service, working for multiple surgeries within a geographical locality. This role includes the assessment and diagnosis of unplanned undifferentiated primary care presentations with the aim to reduce unplanned hospital admissions.- With autonomy to shape the role from onset, identified following up patients helped reduce unplanned hospital admissions as patients or relatives were otherwise calling NHS111/999 during out of hours as felt they struggled to obtain further primary care support and were feeling isolated regarding ongoing management. I then led implementation of this.- Built external relationship with local ambulance service operational management to develop direct referral pathway to enable ambulance crews to contact PPs directly, saving referral time and reducing hospital admissions and A&E attendances.- Implemented and championed use of South East Coast Ambulance Service NHS Foundation Trust's IBIS system to inform crews of complex patient presentations to further reduce admissions out of hours.- Independent Prescriber, able to prescribe non-controlled drugs (due to profession awaiting Misuse of Drugs Act amendment) within scope of practice.- Proactively discuss advance care planning, ceiling of care and DNACPR status with patients, family members and carers with aim to improve personalised care.- Provide clinical telephone advice to colleagues regarding patient assessment, diagnosis and treatment pathways including PPs, GPs and other health and social care practitioners.- Work closely with the Integrated Care Team to raise concerns regarding patients who require input from a multidisciplinary team to resolve a crisis situation or improve quality of life.- Promoted to Paramedic Manager from this role to lead development of PP Team, service and strategy.

- Clinical Facilitator alongside multi professional colleagues providing consultation skill and topic-specific minor ailment training to post-registration Pharmacists.- Lead breakout room education, teaching and encouraging participation from delegates.- Time pressured teaching to stick to schedule whilst also providing personalised feedback.- Fixed term contract ongoing as part of CPCS training schedule.

- Line manager for team of 14 at Walton Ambulance Station of various skill levels from Emergency Care Support Worker to Specialist Paramedic. Managed yearly appraisals, staff development and sickness monitoring.- Undertook performance reviews of staff members within their probationary periods and given underperforming staff to manage across the wider Operational Unit footprint.- Acted as Operational Commander providing first line management response to complex multi-agency incidents.- Acted as SECAmb liaison for the introduction of St Peters Hospital PPCI service, attending monthly meetings as SECAmb representative to identify and manage problems faced with the new service to improve patient access to care going forwards.- Specialist Paramedic in Urgent and Emergency Care working autonomously to assess, treat, refer and discharge patients presenting to the 999 service with urgent or emergency conditions using current evidence based practice.- Worked mainly autonomously on a solo response vehicle as a Specialist Paramedic however also worked as part of dual crewed ambulances and occasionally supernumerary to provide clinical supervision.- Followed up on colleagues' referrals to provide advanced assessment and treatment to patients in community to avoid hospital admission.- Qualified mentor providing support to Student Paramedics.- Administered and supplied medication including analgesia and antibiotics under patient group directives.- Designed and delivered training for clinicians.- Emergency driving trained for both cars and ambulances. Also had off-road driver training.
