(CHS) is a private nonprofit (501 (c) (3) federally qualified health center located at 500 Albany Avenue, Hartford, CT 06120 and 503 Windsor Avenue, Windsor, CT 06095.Annually, CHS serves 19,000+ individuals through 85,000+ visits in Adult Internal Medicine, Podiatry, Optometry, Women's Health, Pediatrics, Adolescent Medicine, Behavioral Health and Oral Health services.
