Rn Care ManagerI'm doing telephonic care management of Medi-Cal members living with AIDS/HIV. This is a pilot Disease Managment Program being administered by AHF. We are an RN based program that is charged with providing Health and Disease Managment support for our members through education, and guidance. There is an RN available 24/7 for members to contact with questions about their disease, medications, resources and health and wellbeing. The goal of the program is simple: member empowerment through knowledge. Through telephonic motivational interviewing, we hope to give the members enough support and education for them to then follow through with what ever process they may need assistance; becoming an active participant in their own health maintenance. By providing this "up-stream" support, the state and AHF hopes to reduce healthcare costs in this population by reducing un-needed visits to clinics, EDs or hospital stays.
