• Completed 62 phone Community Board of Mental Health Satisfaction Surveys• Completed case notes of the 62 surveys into CMIS (Case Management Information System)• Assisted with Alzheimer’s Screening on National Memory Screening Day• Assisted and Observed educational sessions held on-site for the Congregate Lunch Program (Diabetes, Matter of Balance & Caregiver Support Group)• Scanned all of the Illinois Department of Aging and Age Options required forms for case management.• Organized all participant case files to assure accurate, streamlined organization of participation information. • Shadowed a case manager on a community based home visit assessment• Developed an educational flier for local educational and recreational opportunities offered at Concordia, Triton and Dominican. • Who’s Who Binder- I built a binder with short interviews of participants and employees to help build casual conversations and friendships.• Facilitated “Strain Your Brain” Tuesday’s – each Tuesday we would have either an individual game (word search or crossword puzzle) or a group interaction game (wheel of fortune)• Assisted constructing a Thanksgiving “Giving Thanks” tree- individual interviews of gratitude• Constructed a Winter Safety Flier to be distributed to local aging population
