As this hospital does not have a very large ICU, I often had the responsibility of managing critical patients long-term. Worked with Meridian Behavioral Health under the umbrella of BRH on an Assertive Community Treatment team (ACTT) serving Haywood and Buncomb county. As a community-based RN, Case Manager, QMHP, I worked to address many different challenges facing my clients who all suffer from severe and persistent mental illness. I preformed physical and psychological assessments; managed and administered medications, which included: coordination of medication delivery, ordering lab equipment, drawing lab specimens as well as arranging for the tests and reporting results, and filing prior authorizations with insurance companies. I developed, modified, and implemented interventions alongside my clients; provided education and therapy; assisted with activities of daily living; acted as liaison between healthcare providers and clients in the community; navigated COVID landscape to schedule and track appointments, ensure transportation and housing, and communicate pertinent health information to all involved parties; supported clients in accessing community resources, some benefits and education counseling; operated the 24-hour crisis line and intervened appropriately. Personally, I worked to re-structure the existing system in which things were done to make our practice more efficient, useful, and most importantly, safe for the clients.
