Scheduled initial and follow up consultations with members on caseload. Assessed member’s medical and health history, discussed medications, used motivational interviewing to elicit behavior change and motivate members in reaching nutrition goals, and conducted medical nutrition therapy for members in state of licensure. Provided ongoing member support with messaging tool within company platform and resources related to the member’s nutrition and health goals. Maintained thorough record of member calls, health goals and ongoing nutrition plan as well as professional CEUs. Attended weekly and monthly meetings with team as well as communicated with other professionals within company, such as therapists and pharmacists to ensure best care for individual members.

Screened, assessed, and provided medical nutrition therapy for at-risk patients on ICU, multi-service ward, labor and delivery, and psychiatric ward. Participated in interdisciplinary meetings and provided input regarding patient care, including enteral and parenteral feeding recommendations, when necessary. Instructed patients and families on appropriate therapeutic diets to assist in managing medical conditions. Taught monthly classes on general nutrition, heart-health nutrition, and prenatal nutrition. Conducted outpatient nutrition counseling for active military members, retired military members, and family members based on referrals for various indications such as general nutrition, weight loss for service military standards, diabetes, eating disorders, GI disorders, food allergies, pediatric failure-to-thrive, etc.

Provided clinical nutrition services and medical nutrition therapy to clients participating in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). Coordinated breastfeeding support for WIC and managed the Breastfeeding Peer Counselor Program. Supervised lower-level nutritionists as well as breastfeeding peer counselors. Completed administrative tasks such as performance appraisals, timesheets, leave requests, chart audits, and inventoried breastfeeding supplies. Led staff in-service meetings and trainings.

Implemented, monitored, and evaluated Hawaii’s NSLP and FDP. Ensured that both programs were meeting the USDA requirements by conducting reviews and providing technical assistance, training, education materials, and support for each School Food Authority (SFA) on the Hawaiian island chain. Training consisted of meal pattern requirements, dietary specifications, production records, food safety, civil rights, professional standards, school wellness policy, School Breakfast Program, Afterschool Snack Program, and USDA memorandums. Also assisted participating SFAs in spending entitlement monies on USDA Foods and DoD Fresh Produce.
