Provides case management services and supports youth and foster families. Connects and coordinates services between families, agency, government services, and community resources. Documents, reviews, and submits required documentation to the state and affiliated agency. Performs routine visits with families and children as well as safety checks of foster homes.

Monitor the safe houses for issues and provide the first level of supervision and support for the clients.. Complete chore checks each night to ensure the house is being kept clean and oversee the completion of chores. Call and inform on-call staff when there may be a possible issue (when a client comes in late, when a client appears to the under the influence or distress, when there is an emergency situation, etc.) Use de-escalation techniques to maintain order in the home. Alert on-call staff when an issue begins to escalate. Welcome new clients and coordinate with Intervention Coordinator and Survivor Care Coordinators to ensure new clients are welcomed appropriately. Monitor basic house needs (food, toiletries, etc.). Maintain all required documentation, including reporting for grants, in a professional and timely manner in compliance with agency policy and procedures.Communicate, confer with, and coordinate care for clients with other direct services team members. Complete additional duties assigned by the Resident Manager, Resident Coordinator and/or the CEO and/or COO.
