Provide psychosocial interventions and case management services to patient, families, and hospital staff. Bridge patient and families’ community resources to address there​psychosocial needs. Engage and educate patients, families, and on their discharge plans/needs. Assist community organizations with providing services to patients’ while they are inpatient at this facility. Work closely with the interdisciplinary team to ensure the patient understands their plan of care and discharge plans.
