Mental Health Clinician
Current1. Provides direct social work services including crisis intervention as well as individual, family, and group counseling in collaboration with an interdisciplinary team.2. Utilizes therapeutic interventions and evidence-based practices to address patient's holistic social, emotional, and behavioral needs.3. Integrates culturally sensitive and trauma-informed practices and interventions to assist patients.4. Formulate patient-centered and strengths-based individualized treatment plans that leverage the patient's strengths to enhance overall health.5. Conducts comprehensive psycho-social evaluations, assessments, and case management services to determine appropriate levels of care. 6. Facilitates clinical case conferencing with psychiatric providers and primary care team to ensure linkage with appropriate services. 7. Manages referrals and development of discharge plans to support the patient’s needs.8. Completes documentation based on clinical standards, ensuring accurate and comprehensive records.9. Initiated referrals and warm handoffs to necessary services and community resources.10. Collaborate with families and school administration to collect data, implement behavioral intervention plans, and support annual individualized education plans (IEPs).11. Played an active role in department and organizational quality improvement projects.12. Develop and execute outreach activities to identify needs, increase patient engagement and retention in counseling services.13. Utilized knowledge of community resources to refer patients to appropriate services, ensuring continuity and quality of care.14. Participated in weekly clinical supervision and group supervision.15.
