St. Elizabeth Provider Network (SEPN) is a Clinically Integrated Physician Hospital Organization (CIPHO) that aligns employed and independent physicians to improve the health of our patient population by establishing common quality initiatives, economic incentives and care management infrastructure.

Serve in a leadership-triad with the Chief Transformation Officer and Chief Medical Information Officer to lead population health activities at St. Elizabeth. Accountable for the Departments of Ambulatory Care Management, Value Based Performance, Virtual Health, and Quality Payment Programs.

Provide executive operational oversight for more than 250 providers practicing in more than 50 locations in the primary care, pediatric, urgent care, geriatric, palliative care, and business health service lines.Lead population health activities at St. Elizabeth with accountability for the Departments of Ambulatory Care Management, Value Based Performance, Virtual Health, and Quality Payment Programs.

Oversee operations in the development and expansion of the Departments of Care Management; Service Excellence, Education, & Culture; Value Based Performance; Telemedicine; Quality Payment Programs; and Quality Transformation & Process Improvement.Lead initiatives to improve quality and utilization resulting in successful shared savings contract implementation. This includes multiple ACO products, as well as MSSP Track 1 and CPC+, across all lines of business (Medicare, Managed Medicare, Medicaid, Managed Medicaid, and commercially insured patient populations).

In this role I directly support the Executive Vice President of System Development with regard to primary care physician performance while providing leadership and facilitation to the Patient Centered Medical Home (PCMH) Physician Steering Committee. I use my project management and relationship management abilities to effectively and efficiently coordinate TriHealth’s participation in the CMS Innovation Center’s Comprehensive Primary Care initiative (CPCi).My responsibilities include implementing a new primary care physician compensation plan while leading, coordinating, facilitating, and monitoring practice transformation activities. Accountabilities include fiscal management, quality improvement, compliance, communication, and service excellence.I focus transformation efforts and coordinate efforts to comply with and maximize performance in quality measurement, patient and provider satisfaction, cost reduction, and utilization management.

Led TriHealth’s successful application to the CMS Innovation Center’s Comprehensive Primary Care Initiative (CPCi).Managed multiple complex projects working with physicians, payers, community partners, and executive leadership. Served as a liaison between TriHealth CPCi sites, payer partners, community partners, and other health systems.Led project teams through the development of a care management program, implementation of patient/provider shared decision making tools, and other key projects aimed at impacting the triple aim. Coordinated the strategic implementation and expansion of the Patient Centered Medical Home (PCMH) model of care to 34 physician practice locations.
