Round IPH shelters to engage with guests on a regular basis while executing outreach services to community members and non-IPH shelters upon request. Refer program participants to addiction/cooccurring treatment (e.g. inpatient, outpatient, MAT, HIV/Hep C/STD testing). Source and distribute safer-use supplies such as safe sex supplies, wound care, xylazine/fentanyl test kits and Narcan (Folks in need of clean syringes and associated paraphernalia are referred to local harm reduction agencies). Collaborate with community providers, agencies and businesses including public libraries, LEAD, street outreach advocates, neighborhood engagement specialists, first responders, emergency room social workers and coalitions for the homeless to promote a team-oriented approach to care and eliminate barriers to treatment.

Provided comprehensive case management for ten clients via outreach, goal planning, DSS/SSI assistance, medical referrals, transportation, needs assessments and crisis intervention. Established trust and rapport with team members, clients and community providers through consistent communication and actionable goals. Secured suitable housing for a family and single household within the first the two months as a case manager.

Served as a primary support for unsheltered individuals, providing essential resources such as food, water, blankets and harm-reduction supplies during street outreach operations on the Troy van.
