• Rotational post commencing in Trauma, Orthopaedics and Acute surgery in large London tertiary hospital. • Independently carrying a complex caseload across x2 wards, of up to 15 clients with various complex needs from traumatic amputations and ongoing delirium to multiple fractures.• Main responsibilities included; prioritising and independent management of case load; standardised and non-standardised assessments, identifying current and long term difficulties, planning and implementing appropriate acute rehabilitation to enable functional potential and facilitate safe hospital discharge; referrals to appropriate community services for long term needs and rehabilitation. • Working within a large MDT from the therapeutic and medical team, to social and specialist services; to enable client centred care and best practise. • Providing and maintaining Evidence Based Practise and Code of ethics. Locum Occupational Therapist (post registration) and Occupational Therapy Assistant (pre registration)• Independently managing a caseload of up to 12 clients with different conditions ranging from physical and mental health to neurology.• Independently completing ADL/DADL assessments to establish rehabilitation needs, establish SMART goals and organise rehabilitation package of care• Overseeing and carrying out regular reviews to identify change in needs, presentation and adapting rehabilitation plan accordingly • Completing Risk Assessments, identifying ongoing difficulties and signposting to required services within the wider MDT • Assessment of the home environment identifying the need for minor and major adaptations and provision of equipment• Working within a large therapeutic and wider MDT, the client and their family to provide client centred care and community service. • Completing out of hours care; responding appropriately to and attending out of hour calls from emergency services, the client or their family.
