Providing as needed Physical Therapy services in Independent Living (IL), Assisted Living Facility (ALF) and Home Health with focus on Geriatric Rehabilitation. Job duties include, but not limited to: performing interdisciplinary screens on residents for suitability of skilled therapy services, developing functional maintenance programs (FMP) for residents and training facility staff, supervising 1 full time PT Assistant (PTA).

Roles: Director, Staff PTProvided PT services at Skilled Nursing Facilities (SNF) in short-term and long-term setting, and Assisted Living setting with focus on Geriatric Rehabilitation. Responsibilities included but not limited to: assessing functional mobility and providing PT services for patients status post surgeries, general medical, neurological, and cardiopulmonary diagnoses after discharge from hospital, generalize geriatric rehabilitation of debilitated patients, supervising full time PT Assistants; managing patient caseload and daily scheduling; leading care plan meetings and participating in interdisciplinary team meetings, screening residents for suitability of skilled services, developing restorative nursing/functional maintenance program for clients, develop plans for onboarding new hires and provide in-service training for staff and workflow optimization.

Roles: Staff PT.Provided PT services for the 358-bed, specialized Acute Care Hospital & 18 bed Acute Rehab Hospital. Job duties included, but not limited to: Assessing patients’ functional mobility and rehabilitative needs, develop treatment goals and plan of care to assist in their safe discharge; Representing Acute Care Therapy in daily IDT meetings to assist in discharge planning for patients, recommend and promote safe mobility to nursing staff. Worked with patient care in a variety of diagnoses including orthopedic, neurological, general medical, surgical, diabetic-renal, cardiopulmonary conditions and more medically complex patients in the ICU. Worked on data collection and analysis regarding patient care and management during their hospital stay
