The Community Health Services Coordinator works in collaboration with care providers, community members, and case managers to support and advance the health and wellness of newly arrived clients. The Community Health Services Coordinator will assess newly arrived clients’ medical needs, coordinate their initial health screening and refer them to Health and Social Services department.This person is responsible and accountable for navigating care and providing support and logistics for clients accessing care for the first time. The person in this role has the ability to assess health and wellness needs, identify community resources, advocate on the client’s behalf while empowering them with resources to do the same, and is able to identify areas of improvement for service delivery.
