• Performed comprehensive evaluations of patients, including populations affected by strokes, brain injuries, spinal cord injuries, pulmonary diseases, neurological disorders and orthopedic injuries• Scheduled and treated 8-10 patients daily by implementing individualized treatment plans, which were designed to meet the needs and goals of patients, improve their quality of life, and prepare them for integration and function at home and in the community• Collaborated and effectively communicated with staff and the rehab team through daily conversations and weekly team meetings, regarding patient progress, needs, and plans• Tracked and monitored patients’ progress with Functional Independent Measure scale and other standardized motor, cognitive, visual, and functional assessments• Assessed and trained patients for necessary ADL adaptive equipment and durable medical equipment• Maintained complete, clear, and accurate daily documentation including progress notes, plans of care, and discharge paperwork• Prepared patients and families for discharge by providing them with training, resources, and an occupational therapy home program
