Key Responsibilities- Manage and provide positive and active leadership to Referral Coordination team (including rostering, mentoring, coaching and performance management)- Ensure the Queensland Strategic Plan is implemented according to established timeframes and expectations- Liaise directly with the Police Project Team (QPS), providing prompt communication and detailed information as requested- Ensure that the service runs efficiently and effectively, to exceed agreed Service Agreement targets and key performance indicators, according to best practice standards- Manage relationships with 25 key State/ National partner agencies across multiple sectors (Domestic & Family Violence, Mental Health, Victim Support, Migrant Support, Alcohol and Drug Misuse)- Comprehensive knowledge of the IT platform and ability to use high-level functions to capture data in all aspects of Referral Management plus ability to on-train these attributes- Regularly liaise with the Assistant Director for Quality Assurance and IT to advocate for QPS and partner agencies and continuous improvement of the referral management system- Prepare reports and proposals as required (for SupportLink management, QPS or other agency partners)- Prepare detailed monthly reports to the Assistant Director and CEO outlining referral numbers, performance by region and strategies trialled and to be implemented by the team to increase referral making- Represent SupportLink at senior level meetings with QPS, Education QLD and non-government organisations- Workplace Health and Safety Officer and Quality Management Supervisor for Queensland site office

- Work within quality and risk frameworks and guidelines to ensure high quality service delivery. Focus on respecting client's rights and confidentiality- Review referrals made by Police, Ambulance, Schools, Probation and Parole and ensure that referrals are sent to the most appropriate support agency in the local area for each client- Extensive knowledge of a Police Region is required regarding services available, dynamics and demographics of the region, volume and nature of referrals- Liaise with 89 partner agencies bi-monthly to monitor capacity to respond to referrals, service updates and changes- Advocate for clients referred by Police to ensure they are receiving appropriate responses and intervention- Identify service gaps and develop partnerships with local agencies (including a formal Agency Agreement) to ensure that Police can make referrals for any person at-risk and they will be linked with support- Extensive knowledge of the SupportLink IT platform for referral making. This included providing training on a regular basis to new Police/ agencies either in person, by phone or via webinar)- Promote referral making (by Police/ Ambulance/ Schools) through developing strong relationships with operational officers as well as staff at a management level- Provide support to less experienced staff in an official mentoring and coaching capacity- Knowledge of multiple sectors of service delivery (aged support, disability, housing, homelessness, drug & alcohol, youth at risk, family support, domestic and family violence, victim support, Aboriginal and Torres Strait Islander issues)

I was the Client Service Delivery Coordinator for the Youth Services Team which comprised of the following programs- Youth Support Coordinators (YSC)- Young People's Community Health Access- Volatile Substance Misuse (VSM)- Youth At Risk Outreach Service (YAROS)- Youth and Community (YACA)It was my role to report back to ensure that each program was meeting it's requirements and that the service provided to at-risk clients and families was of the highest possible standard. I provided regular case review meetings and team supervision, and led these teams to provide the individual support, group work and community development and education each program was responsible for.
