Hazel Lambert is the Director of Riverside County Long-Term Care Ombudsman Program. She previously served as the Program Manger for San Bernardino County Long-Term Care Ombudsman Program. She has been a State Certified Ombudsman for over 15 years. She has been recognized and awarded the Outstanding Area Ombudsman for San Bernardino, and has an established reputation advocating for policies and practices that promote and improve health equity for underserved populations.Hazel is a seasoned professional skilled in representing stakeholders to leverage resources and works to build collaborative relationships with local, civic, and state agencies. She holds a Bachelor's degree in Business and provides training and consultation through her small business Advocate for Nurturing Transition Solutions (ANTS).Hazel has established a name that is well-respected within her community. She has served in multiply capacity on local boards and committees while holding herself to a high standard of excellence, integrity, and accountability. Her work centers on building capacity through education, awareness, empowerment and community engagement.Hazel is an effective leader, advocate and entrepreneur. She is concerned about the needs of the underrepresented population within her community including the elderly, disabled, children, families and youth. Her mission is to advocate for nurturing transitional solutions that support positive changes through state legislation, education and community outreach.
