Current• Unit Based Pharmacist: Emergency Department & Inpatient Specialized Units:o Verification and clarification of medication orders.o Participating in daily multidisciplinary team (MDT) rounds.o Supports ED Stroke, STEMI & Code teams.o Review of patient medication profile.o Performs routine pharmacotherapeutic monitoring .o Coordinating discharge medications with discharge pharmacist and others involved in discharge process.o Medication counseling upon patient discharge.o Resolving any medication related issues (such as drug outages, medication administration, missing doses, etc.)o Solving technical issues in ADMs: Pyxis• Investigational research drug services pharmacist:o Leading & coordinating Investigational drug services.o Participates in study initiations, monitoring and closeout visits, quality assurance audits and other related meetings with study Investigators, sponsors, regulatory agencies and others.o Manages recordkeeping, receiving, storage, retention/destruction and related inventory activities in compliance with good clinical practices and the requirements of individual study sponsors.o Medication order built & validation (Epic research module) in collaboration with information technology department.o Patient randomization in randomized controlled trials.o Oversees investigational medications dispensing and sterile products preparations following proper aseptic technique.• Preceptor: ASHP accredited pharmacy residency program, pharmacy internship program.• Antimicrobial stewardship program (ASP) pharmacist.• Supervises the activities of pharmacy technicians related to the timely, accurate preparation and delivery of medication orders.
