As its name suggests, AccessHealth was established to help the uninsured and financially vulnerable gain access to important health care services they need. Additionally, through care coordination, navigation and case management, our team helps to address the social determinants of health which account for 80% of the barriers to health. Through a network of medical providers and community partners, we are able to improve patient engagement, access to care, appropriate utilization of services, and long-term health outcomes. I have served as program director of the AccessHealth program serving Oconee and Pickens Counties since 2013. In January 2020, I assumed responsibility for the AccessHealth program serving Greenville and Laurens Counties, as well. Our team assists patients in these four counties and select communities in bordering counties. As a program of the Accountable Communities Department of Prisma Health, our sister programs serve as critical partners in the support of our patients. Show less
