Work directly with Hospital Case Managers, Social Workers, Doctors, Cardiology, Neurology, & Rehab Specialists, Home Health & Hospice Agencies, and Primary Care Doctors assessing the needs of patients seeking placement in, and transitioning to, their next level of care in a skilled nursing facility. Assessment includes: the patient's current medical condition & needs, if our skilled nursing facility can meet those needs, what the patients payer source is (Medicare, Managed Care, Medicaid, Self) & what those benefits are.Constant communication is kept between case manager and/or social worker, discharge & admitting facility, patient, patient's family and/or power of attorney, primary care physician, Facility Administrator, Director of Nursing, Business Office Coordinator, Rehab Manager, Facility Social Worker, nursing staff, aides, van driver, Dietary Manager and all individuals involved in the physical, medical, emotional, and financial care of patients going to and from our facility. Position involves sharing the facility's ability to admit or not to admit, based on the criteria set before by Medicare/Medicaid/Managed Care policies, as well as the facilities ability to meet the needs of the patient & availability. Encouragement and understanding are a necessity in this position, due to the intensity of the situations patients and their families are facing. Education of patient benefits & application of those benefits is also a key role of this position.
