Adults Program Specialist- provide policy analysis and recommendations for program adoption and implementation based on state and federal regulations and guidance communications. Implantation analysis to impact on business practices, crossover into other units, training needs, fiscal impact and review for alignment with HSA mission. Project Management- mange projects necessary to bring in new social services programs, revise existing programs to align with changes in regulation or internal business processes to maintain alignment with regulations and mission/values of the agency. Implementation of recommendations, status reporting related to milestones and deliverables, unanticipated barriers, risks and mitigations implemented. Maintain project schedule and lead main and sub workgroups through work product deadlines for milestone/deliverables. Aid with development of project implementation and ongoing program budget within allocation amounts and allowable parameters. Identify potential gaps in service delivery or workflows and work with implementation partners to provide for training, workflow revision or other necessary mitigations to achieve identified performance indicators. Contracts/Contract Management- provide analysts of existing contracts including vendor progress in meeting performance indicators. Recommendations for adjustment of budget, incentives, or Request for Proposal/Information. Authorize contract payments, reconcile invoices, and maintain expenditure reports for contract year and contract term. Contract procurement with negotiations, scope of work development and benchmark definition. Facilitate review and vetting through program, executive leadership, purchasing, Risk Management, legal counsel, and Board of Supervisors. Complete document requirements including Sole Source, Sub-Recipient determination and Board Agenda Items to meet contract deadlines.

Visalia-Hanford Area

Provide Intensive Case Management for the aged and medically frail population at risk of premature nursing home placement. Provide regular in person, and telephone services. Conduct a variety of assessments to create individual service plans focused on person centered needs, goals, and interventions. Monitor effectiveness of service plan interventions and progress toward meeting identified goals, address barriers to provision or implementation of interventions to mitigate barriers and achieve goals. Assist with identifying and offering harm reduction interventions as needed. Budget, authorize and reconcile individual service plan spending to provide intervention services within program specific thresholds. Act as the informal lead worker for the unit by providing coaching, mentoring and training. Approve time off requests, intervention requisitions, and service plan authorizations in the absence of direct supervision. Approve time logs for interns and temp to hire staff. Participate in panel interviewing of potential candidates, interns and temp to hire staff. Serve as the informal subject matter expert for program specific requirements. Advocacy for staffing support to meet program obligations which unmet would have negative impacts to funding availability and use. Program policy interpretation and internal policy development. Participation in state and federal program audits, findings resolutions and development of formal corrective action plans. Complete and submit required program reports to oversight agencies related to spending, caseloads and staffing levels.
