Current• Experience with multiple medical populationso July 2014-June 2015: Work primarily in Trauma/Ortho/general-surgeryo June 2015-Present: Work primarily in Emergency Department• Complete psychosocial assessments• Complete virtual follow up calls with patients +COVID who were discharged from inpatient setting during height of COVID pandemic• Complete mental health assessments, complete CSSRS assessments, coordinate transfers to psychiatric hospitals, complete MOT, utilize XFERALL for mental health transfers, provide outpatient resources• Provide discharge planning services to patients• Provide crisis intervention services to patients• Provide referrals and community resources for patients• Participate in weekly and biweekly huddle team meetings• Participate in weekly resource management meetings• Comply with HIPAA, EMTALA, and Medicare guidelines• Complete mandated reporting for APS/CPS/DARS/HHSC and Duty to Warn with law enforcement• Communicate effectively with nurses, doctors, patients, multidisciplinary teams, supervisors• Communicate effectively with community agencies such as: psychiatric hospitals, LMHA, SNF, home health, hospice, DME providers, LTAC, EMS• Communicate effectively with community resources such as: shelters, law enforcement, mental health court, justice of the peace, LMHA, homeless services• Complete documentation promptly• Utilize “Epic” program for documentation• Develop and/or maintain tracking tools for Ocean’s patients seen in ED/hospital, mental health patients seen in emergency department for RAC, expenses • Participate in Diversity Equity & inclusion Council• Interview, train, supervise masters level interns• Available as needed, help team when short staffed, cross trained on all floors
