• Conducted psychosocial intake assessments via phone to introduce the iCBT program, assessed clients’ fit and risk, engaged clients and solidified their commitment to the program• Offered iCBT to a wide variety of clientele including delivering psychoeducation and module feedback • Performed case management duties such as providing supportive counselling, problem-solving to address any barriers to program completion and facilitating referrals to appropriate community resources • Monitored clients’ progress and adjusted treatment plans as required to facilitate improved outcomes

• Provided coverage when required for all units at Providence Healthcare (stroke, geriatric, orthopaedic and amputee rehabilitation and palliative care) to offer support to clients and continue with case management activities while social workers were away • Located and built rapport with a variety of community organizations to develop long-term relationships and offer additional supports to clients and team members• Assisted patients in completing a variety of financial and housing applications, such as Housing Connections, Ontario Disability Support Program and Canada Pension Plan, to address their needs

• Engaged and created professional relationships with clients who, most often due to a diagnosis of dementia and related behaviours including aggression and paranoia, required a secure hospital unit• Provided counselling and psychoeducation to clients and their families, to support them through the progression of dementia and address challenges including feelings of loss, guilt and anger• Liaised with specialized community resources, such as behavioural support specialists, and implemented recommended interventions to address clients’ behaviours and facilitate their transitions into appropriate discharge locations in the community • Conducted capacity evaluations and utilized appropriate legislation, such as the Substitute Decision Maker Hierarchy in the Health Care Consent Act, to obtain informed consent related to discharge
