• Identify, establish, and maintain professional relationships with targeted hospital systems, including Swedish, UW, Virginia Mason, Evergreen, Skagit, and PeaceHealth.• Show expertise in local market dynamics (competition, payers, therapies) as well as complex disease states Option Care Health services.• Demonstrate effective selling skills during physician/ office staff presentations with existing and potential customers.• Consult with clinicians as well as medical office staff to appropriately promote Option Care Health and provide industry-leading customer service.• Review and analyze all available sales data through Salesforce and Playmakers Edge, to create effective territory plans and utilize promotional budget funds.• Maintain face-to-face interactions with healthcare providers, focusing on top target customers to achieve all sales performance goals and objectives for geographical responsibility.• Results-oriented mindset with expertise in the complete selling cycle (call objectives, opening statements, listening, use of questions, feature & benefit statements, use of visual aids, objection handling, closing, post-call analysis).

Service as a key point of contact and representative to provide education, assistance, and service to referral sources. • Review patient’s medical record to obtain both pertinent medical history and primary/secondary insurance payer information and communicate to registration department. • Participate in any data collection required for therapy start and patient tracking process.• Provide hands on, in person education to patients and their caregivers with the goal of timely discharge and therapy independence. • Provide oversight and input to the providers regarding the patient and proper assessment and treatment process and transition to home care. • Partner with Account Manager and Regional Sales Director to create and execute area business and growth plan.

• Obtain baseline data on patients including histories, vitals, lab tests and health education.• Ensure timely and accurate documentation of patient care observation and activities.• Provide ongoing education to patients regarding health and psychosocial concerns. • Evaluate patient knowledge regarding health, illness and self-care practices; educate patient on positive lifestyle and health choices. • Proficient in utilizing EMR system for documenting patient care and reviewing records for health maintenance needs, order entering, medication refill requirements, scheduling appointments and processing in-basket messages. • Perform telephone triage; assess patient issues and advise to ensure optimal outcomes.

• Assist the Director of Nursing in planning, directing, evaluating nursing departments, and implementing plans of actions to correct identified problem areas. • Collaborate with the Interdisciplinary team, regarding the progression of the patient throughout stay. • Discuss discharge planning including home healthcare and DME needs, as well as follow up physician appointments during Utilization Review meetings on weekly basis.• Review the outcome from home evaluations and recommend any necessary needs to ensure the patient is safely discharged back to their homes. • Participate in managed care meetings daily for skilled residents on the unit. • Assist in the completion census information and monitor all charting of nurses for compliance with State and Federal regulations (assessments and plan of care).• Collaborate with the HR department in the hiring process, from scouting new potential hires, interviewing potential prospects, and assisting in training new hires.

• Direct and supervise all unit staff in nursing care. • Establish a therapeutic relationship with residents and families.• Ongoing interaction with physicians and pharmacies regarding residents’ conditions and treatments, participating in developing, implementing, and evaluating the residents’ plan of care.• Operate clinic hours for independent residents daily regarding consultation, injections, wound care, etc. • Process admissions/discharges paperwork within the nursing scope. • Compile monthly summary for risk events.

• Medical/Surgical Unit on the LTAC floor where there are high acuity of ventilator dependent patients.• Care and practice related to patient care needs involving various types of wounds, ostomies, etc. Daily use of alternate routes to administer medications other than by mouth (ex: PEG, NG, PICC).• Provide planning and delivery of direct and indirect patient cares through the nursing process of Assessment, Planning, Intervention, and Evaluation. • Develop nursing care plans in coordination with patient, family, and interdisciplinary staff as necessary. • Communicate changes in patient's clinical condition with Physicians, Nursing Supervisor/ Manager, and co-workers as appropriate. • Participate in admission and discharge planning processes.
