The Performance Excellence team leads, educates, and supports all clinics and hospitals in our enterprise with the utilization of all best practices that align with our Customer Service Standards. The Executive Director is responsible for the overall coordination and administration of the Hospice program including growth and financial performance.

The Director of Business Development develops relationships with managed care organizations, hospitals, nursing homes and other healthcare providers. The Director will also manage the daily operations of the Referral and Admissions departments and is responsible for supervision of the Hospice Care Consultants, Referral Department Staff.

The Hospice Care Consultant interacts and assists with referrals from healthcare professional in hospitals, home health agencies, nursing homes, long term care facilities and physicians in private practice. The success of this position derives solely from the provider relationships that result in the admission of appropriate patients from physicians, discharge planners, case managers, floor nurses, and other healthcare professionals. HCCs participate in interdisciplinary team meetings and weekly marketing meetings as assigned by the Executive Director and or the Director of Business Development.

Plan, organize, direct and manage all aspects of the hospice program, including inpatient, residential and home hospice.Ensure consistent practice of the hospice philosophy within all corporate sites.Assure budget standards are met and programmatic standards are achieved.Build volunteer program within residential and facility-based hospice.Provide in-servicing to all contracted providers.

Created innovative program for interdisciplinary electronic documentation resulting in deficiency-free state survey.Developed Creative Arts Therapy referral program.Started national roster music therapy internship program.Grew volunteer program to over 425 active volunteers.
