Provide support services to and on behalf of, adults with developmental disabilities including assessment of the person's community support needs, developing individualized support plans, and coordination of ongoing services. Maintain a caseload of up to 25 individuals with developmental disabilities, mental illness, sensory issues, and physical disabilities.Conduct intake and ongoing assessments of clients on caseload.Develop collaborative relationships with support staff, family members and guardians, as well as various provider agencies in order to assure quality supports are developed and maintained over the long term.Work in conjunction with service providers and DCHS to advocate for clients health and safety needs and enhance each client's quality of life. As a member of the Senior Management Team, work with colleagues on developing and maintaining training curriculum for all employees at CLC.Develop and implement tools to assess and improve quality of supports for the individuals receiving services. Develop and conduct agency-wide training on topics such as Person-Centered Support, Abuse and Neglect, Confidentiality, and Supporting Individuals with Challenging Behaviors. Supervise, train, and support members of management as well as direct support staff.
