Coordination of research and program implementation activities with participating aged care services, including eligibility and readiness scoping and assessment, organisational change culture assessment and management, facilitation of partnerships, and ongoing coaching and support through the improvement phase. The focus is improving the quality of palliative care and advance care planning in our aged care sector through auditing, education, upskilling of workers, and working effectively with others.
