Senior Clinical Pharmacist
Current* Lead for several outpatient pharmacist clinics (IBD, rheum, derm) working closely with physicians, primarily serving patients on immunosuppressant and/or biologic to manage their autoimmune/ inflammatory conditions - optimise medication therapy for patients by dose titration (treat-to-target) and monitoring efficacy and safety of treatment- provide therapy and disease counselling- resolve drug-related problems including adherence, side effects, untreated indication etc - ensure patients are uptodate with their health maintenance eg vaccinations, bone health* Set up allergy pharmacist clinic that sees patients with beta lactam and NSAID allergies - reduce wait time for first visit for patients with query / reported allergy to beta lactams or NSAIDs - conduct extensive history taking to formulate clinical assessment, determine appropriate skin testing and /or oral provocation tests , and interpret findings to ascertain allergy status - delabel penicillin allergy where appropriate to reduce inappropriate antibiotic use, potentially improve clinical outcomes as well as reduce future healthcare costs * Participate in hospital formulary management as Pharmacy & Therapeutics member - perform drug reviews and drug use evaluations- formulate prescribing guides for newly approved drugs- gather feedback and inputs from clinicians on formulary decisions- encourage cost-effective therapy selection at the hospital level
