I am currently undertaking a rotation on the Orthopaedic Trauma and Fractured Neck of Femur wards as part of my Band 5 development. My role consists of: - Performing Day 1 Post operation assessments : joint range of motion, muscle strength, mobility and transfer practice.- Rehabilitation: adopts a biomechanical frame of reference in particular for total hip replacements. This enables me to teach safe transfer methods to build a patient's independance within meaningful ADL's. Exploring the use of assistive equipment to encourage funcitonal independance and confidence. - Mobility practice: stairs assessments, prescription of mobility aids.- Mental Capacity Assessments in relation to discharge planning.- Discharge Planning: assessing and completing HNA's for appropriate discharge pathways- equipment ordering and risk assessment, fire safety referalls, community rehabilitation and MSK outpatient referalls.
Occupational Therapist - Stroke Rehabilitation
I am currently working as a Band 5 Occupational Therapist within a Step Down, Stroke Transitional Unit. As part of my role, I perform comprehensive initial assessments to identify cognitive, visual, sensory, proprioceptive, strength and movement deficits in order to establish a holistic, person centered rehabilitation program for each one of my patients. I play a vital role within the discharge planning process, ensuring that patients are able to return to a suitable environment by assessing patients within both the ward and home environment (e.g. home visit, D2A) and providing adaptive equipment for them to ensure they thrive.I am developing diverse approaches to upper limb rehabilitation including techniques such as mirror therapy, passive and active assisted functional movements, sensory re-education and integration, joint position sense, joint mobilizations. From a cognitive rehabilitation standpoint, my role leads me to deliver cognitive interventions in order to improve short and intermediate memory using chunking techniques.My role adopts a compensatory approach to visual rehabilitation following stroke, including the ability to increase scanning ability and integrate visual information whilst mobilizing within the environment.

Becoming a part of the Leg Club MDT during the pilot phase of this scheme allowed me to assume multiple responsibilities including: - Supervised delivery of therapeutic exercises to improve range, strength, proprioception & endurance- Exercise tailored to each patients needs and ability- Performance of outcome measures (e.g. ankle range of DF/PF, Timed up and go/ Sit to Stand/ FADI/ Quality of life questionnaires). The Leg Club involved a variety of session bi-weekly focusing on patient education, therapeutic exercise prescription, nutritional education for chronic wound healing and promoting positive behaviour change. This new upcoming scheme boasted an 80% healing rate amongst participating patients suffering from leg ulcerations.
