Supportive Palliative Care Social Worker
CurrentConducts sessions with patients experiencing life-limiting illness and their loved ones to complete assessments, develop treatment plans, provide supportive counseling, crisis intervention, and care management. Routine interventions include advance care planning, continuum of care, end-of-life counseling, family meetings, referrals to community resources, new diagnosis or adjustment to illness counseling, bereavement support, and patient advocacy. Collaborates with interdisciplinary teams and involves patients in the development and implementation of care plans.
