: As a Support Worker for vulnerable adults and individuals with learning disabilities, I am responsible for conducting assessments and updating person specific support plans and care duties. Established a therapeutic relationship with individuals, gained their cooperation in situations when there may be significant barriers to understand due to the effects of their illness.

Prepared, maintained and updated up to date support plans and co-ordinated the provision of high quality, support staff. Ensured all current and newly referred individuals received high quality, person centred support. Assisted the Manager with all regular duties.
