Als POH GGZ zie ik patiënten met psychische klachten. Door middel van kortdurende interventies en gesprekken probeer ik de patiënt verder te ondersteunen en begeleiden in de hulpvraag. Wanneer/ waar nodig ga ik samen met de patiënt op zoek naar een geschikte plek om de patiënt naartoe door te verwijzen.

Geregistreerd Jeugdzorgwerkers (sjk)Van pedagoog getransformeerd naar trajectbegeleider; van behandelgroep naar jongeren ambulant begeleiden die terug kunnen vallen op hun steungezin. Ik begeleid de jongere naar het zelfstandig wonen en werk daarin nauw samen met het netwerk van de jongere. Coachen, stimuleren, begeleiden en sturen staat centraal in mijn functie als pedagogisch traject begeleider. Als zorgcoördinator vormde ik samen met gedragswetenschapper de aanmeldingscommissie. Hierbij zijn zicht hebben op duur van lopende trajecten en mogelijkheden voor nieuwe trajecten en kennis van de sociale kaart ivm doorverwijzing de kerntaken. In 2015 is hier de functie accountmanager bijgekomen, waarin het leggen en onderhouden van contacten met gemeenten en (zorg)instellingen veel meer centraal is komen te staan. Door de transitie is het belangrijk om het SGJ aanbod onder de aandacht te brengen bij de doorverwijzende instanties en ketenpartners. Wij Samen is daarin wat ik belangrijk vind; samen zorg dragen voor de jongeren.Door de fusie tussen SGJ en Timon zijn de functie van zorgcoördinator en accountmanager voor mij komen te vervallen.
