Duties: Asses patient location, activities, environment and mental status of patient every 15 minutes and record in log book daily, assist patient with grooming, bathing and feeding daily. Transport patient to daily activities, dentist, physical therapy, and doctors appointments. Perform and record vital signs daily, provide one to one care to patients that are high risk for suicide ideation,report any changes in patient physical or mental status to nurse or charge nurse, and closely monitor patients while eating in dining room or tv room daily.

Duties: Assist nurses and doctors with providing nursing care to patients in a short term care facility, assist patients with bathing, grooming and feeding. Measure and document patients input and out in daily log book, report any abnormal findings to registers nurse and LPN, transport patients to physical therapy and rehab.
