    After 15 years of watching SNF portfolios write off the growing cost of HMO denials, poor care coordination, premature discharges, and medical malpractice lawsuits that were all a result of poor or missing documentation issues I set out to create the solution I always wished existed.In 2023 CMS.gov published they will be using AI to screen through submitted claims to reduce the $110 billion of medical care payments they they identified as incorrectly billed per year due to “medical records that do not support the diagnoses submitted to CMS, or the risk scores may be inaccurate and result in payment errors.” Even in cases when the necessity may have existed, but the documentation failed to prove it leaving the burden on healthcare providers to pay the price of their missing documentation.
