As an Access Facilitator, my role involved connecting with families to discuss their respite needs. Respite options were provided either internally, through our CHAP (Community Helpers for Active Participation) program, a program that facilitates the connection of individuals with disabilities and support workers, or externally through connecting the families to various Toronto social service agencies that provide out of home options.Met with families to assess their respite needsConnected families to respite options in the GTAAssisted families in acquiring funding options including ministry fundingPartnered with social service agencies located in the GTA in order to provide families with options beyond the CHAP programRecruited, screened and provided orientation for potential respite workersFacilitated the match of respite worker and familyAssisted in maintaining the worker and family connectionParticipated in individual case conferences with social services professionals such as: social workers, teachers, doctors, principals, psychiatrist and behavioural consultantsAdvertised recruit opportunities using job search sites such as Charity Village, local papers and community colleges and universities
