Care Manager
CurrentPersonal case load of 40 families including Basic HCBS Plan Support. Responsibilities include initial & ongoing assessment of client strengths and long-term needs, informing families of service availability, assisting families with program applications, and serving as a family advocate for continued access to resources.• Maintain strong relationships with families to provide community resource information and advocacy needed to aid in accessing these services. • Develop ISP/Life Plans with clients with involvement of family, relatives, and service providers.• Assist qualifying families with medical services, HCBS Waivered services, and transitions between school and day habilitation or vocational programs.• Assist families without Medicaid in obtaining Medicaid-Waiver, as well as gathering all documents needed to meet Office for People With Developmental Disabilities (OPWDD) requirements • Assist Medicaid families in applying for Social Security Income and other appropriate services through the Department of Social Services including Medicaid-Waiver, Supplemental Nutrition Assistance Program, and Housing Program Section 8.• Maintain strong communication with supervisor, as well as up-to-date documentation and client file organization to ensure continuing access to services for the client. • General office duties and frequent work with Microsoft Office.
