I am a dedicated Pharm D graduate from Dr D.Y. Patil Institute of Pharmaceutical Sciences and Research, Pune and gained invaluable experience through a one-year internship at Dr D.Y. Patil Medical College, Hospital, and Research Centre. Recently, I further honed my skills during a three-month internship as a Clinical Pharmacist at Apollo Hospital, Navi Mumbai.During my internships, I played key roles in:Participating in ward and ICU rounds, where I reviewed patient profiles and charts to ensure the appropriateness of medication orders, assessed drug allergies and drug-drug interactions, and identified therapeutic duplications to promote safe and effective drug therapy.Reporting and analyzing Adverse Drug Reactions (ADRs) and medication errors to minimize their occurrence and enhance patient safety.Counseling patients about their diseases and medications to ensure they understand their treatment plans.Assisting with operational tasks, including medication management and monitoring medication expiry dates.Contributing to the Antimicrobial Stewardship Program, ensuring the appropriate use of antibiotics.Reviewing and analyzing chemotherapy protocols and adjusting the dilution of cytotoxic drugs based on patient-specific dose requirements.Completing a project on inpatient medication reconciliation, improving the accuracy and safety of patient medication records.With a strong foundation in clinical pharmacy, I am committed to advancing patient care through safe and effective medication management.
