In this experience, I worked as a facilitator and advisor for the elderly population of a Long-Term Institution for the Elderly, evaluating their daily medications, according to their medical records and needs;After carrying out a case study at the institution, using the Beers Criteria and the Micromedex platform as tools, I realized that more than 80% of elderly residents were using inappropriate medications and had drug interactions in their prescriptions, increasing the risk of… Show more In this experience, I worked as a facilitator and advisor for the elderly population of a Long-Term Institution for the Elderly, evaluating their daily medications, according to their medical records and needs;After carrying out a case study at the institution, using the Beers Criteria and the Micromedex platform as tools, I realized that more than 80% of elderly residents were using inappropriate medications and had drug interactions in their prescriptions, increasing the risk of adverse reactions and worsening of their health conditions;After an expansive literature review for safe alternatives to inappropriate medications and management of drug interactions, such as adjusting the time, and dose or replacing the medications used, I proposed an intervention aimed at prescribing doctors who monitored the elderly in institutions. I also presented the results of this research to the municipal Health Department, proposing the inclusion of safer alternatives for the elderly in the municipal list of medicines made available by the public health system;As a result, the institutions received a reinforcement of stocks of medicines considered safe from the Municipal Health Department, in addition to changes in the pharmacotherapies prescribed by doctors that guaranteed the minimization and mitigation of adverse reactions in the elderly related to the use of medicines.
