The Northern Specialist Family Violence Service (NSFVS) is the lead provider and access point for the integrated family violence service system across the Hume Moreland region and provides specialist family violence case work across the North East Metropolitan Area of Melbourne as well as the Hume Moreland region.In this role I provide Specialist Family Violence outreach case work to victim survivors of family violence and their accompanying children. Key components of this role include collaborative risk assessment and safety planning according to the Multi Agency Risk Assessment and Management (MARAM) framework, and casework aimed at promoting victim survivors safety and recovery from the impact of family violence and increasing perpetrator accountability.Underpinning this work is the commitment to an intersectional feminist framework, recognising the gendered nature of family violence and that an experience of family violence is further compounded by other (often multiple and intersecting) experiences of oppression.

MARAM Collaborative Practice Sessional Trainer

J2SI is a long-term support program for people who have experienced chronic homelessness. J2SI provides up to 3 years of support per participant in a trauma informed, relationship based model of care.J2SI Phase 2 was part of a randomised control trial, where the intervention (our support) aimed to build upon existing evidence to create effective support programs for highly-marginalised people who has a long term experience of homelessness. This research has contributed to a robust case management model that has proven effectiveness to end homelessness and reduce associated costs.Through J2SI I supported individuals and families to rebuild trust through, secure and maintain appropriate housing, connect with community, engage with economic participation activities, address health and wellbeing goals and (re)build independence.

The Community Connections Program (CCP) is a Health and Community Care (HACC) funded program. Through my work in CCP, I provided support to people experiencing unmet, complex health needs and cooccurring homelessness or housing difficulties.While working with CCP, I also coordinated a monthly Health Time event at a local public housing estate. Through Health Time, we successfully linked vulnerable older populations to relevant health and support services and nurtured a sense of community at a large public housing estate in Moreland City Council.

For approximately 6 months during 2014-2015 I acted in the position of Team Leader of Outreach Support. During this time I was responsible for: recruitment and induction of 2 positions; supervision of ten staff across the Crisis Support Program, the Community Connections Program, Accommodation Options for Families, and Access and Support Care Coordinator; all reporting, financial, day-to-day management and administration requirements.I also participated in Specialist Homelessness Services accreditation and planning and change management relating to position and team design changes while acting in this role.

ConnectED is part of Alfred Health’s Hospital Admissions Risk Program (HARP). Through my role in ConnectED I provided outreach case management to people with the aim of reducing avoidable Emergency Department (ED) presentations and hospital admissions. Through my position with ConnectED I worked across Alfred Health sites in a multidisciplinary team to provide high quality support to people experiencing multiple and complex health and social needs. I was responsible for creating thorough ED management plans for each of the people I was supporting through ConnectED to ensure a coordinated response by hospital staff.

In the LinkOut program, I provided pre and post release transitional support program for men exiting prison. Link Out was a program targeted at offenders approaching their release date from custody who were assessed by Department of Justice to be a moderate to high risk of re-offending and to have multiple and complex support needs.The clients I worked with in Link Out often had support needs including but not limited to: histories of childhood trauma; substance misuse; mental health difficulties; child access restrictions or involvement with DHS child protection; cognitive impairments (in particular Acquired Brain Injury); histories of multiple incarcerations; histories of homelessness; limited employment prospects; challenges with daily living skills; cultural and/or religious needs; a possibility of negative media attention.
