I fly across Victoria and South Australia to provide OT services to rural and remote communities.

I work with aged care clients to help them stay safe and active in their homes and with complex mental health conditions to help them identify and build meaningful habits and routines.

I provided home based care for older persons whereby my role was to help them to maintain their independence and safety in completing daily tasks whilst within the comfort of their own home and to encourage engagement within the community.Key responsibilities:- Personal care including medication prompting, showering, dressing, compression stockings, puffer administration.- Social support including transporting clients, taking clients to community classes, assisting with grocery shopping, providing social interaction.- Domestic assistance including basic house cleaning and household duties.

I completed a nine week placement split across the Elizabeth clinic, city clinic and Noarlunga clinic. During this time I shadowed three occupational therapists and was exposed to a wide variety of occupational therapy contexts and interventions. I gained experience writing NDIS reports.

During my time in Port Lincoln I participated in a Participatory Community Practice Placement whereby I completed a community project focussed on the mental health referral pathway. I spent time connecting with mental health service providers across the scale of risk and severity and GP's within the region. My partner and I created a referral pathway flowchart for mental health presentations which is now used within the Port Lincoln and Mount Gambier GP clinics.

I completed a 9 week clinical placement in the Geriatric and Evaluation Management Unit (GEMU). My role was to collaborate within the multi-disciplinary team to safely discharge patients home from the hospital. Here I completed initial and functional assessments.Working under two occupational therapists with limited supervision. Key responsibilities:• Standardised assessments such as initial assessments and functional assessments; personal care and kitchen assessments.• Provide assistive equipment to enhance safety at home. • Liaise and link patients with services and agencies within the community.• Provide patient and family education regarding falls, energy conservation and hip precautions.• Involved in the assessment of patients physical and cognitive function and home situation.• Referred patients to allied health professionals to improve function, maintain function and prevent deconditioning.• Involved in multi-disciplinary team meetings to discuss patient progress and discharge planning.
