Currently an advocate and educator for senior health and wellness support, I have varied experience in the hospital setting including medical/surgical, Pediatric Emergency and Trauma. Obtained Certification in Case Management in 2012 after working with a talented and diverse team of Case Managers and growing exponentially during the experience. 2013 brought the opportunity to participate with Arizona Care Network, a Phoenix Accountable Care Organization with Dignity Health and Abrazo Healthcare. This experience has afforded the opportunity to mature and refine skills relating to long term care, Geriatric nursing, HEDIS quality measures, Community nursing and Social Services. Taking advantage of this opportunity, I am applying innovative concepts, as well as deepening relationship skills and philosophies every day as I strive to make appropriate health care available and understood by eligible patients and their families. In an effort to contain organization costs, the opportunity to perform multiple roles and expand leadership experience including assisting with organization goal achievement, peer coaching and new hire training, has encouraged my sharing of expertise for clinical technology systems and contributing to process and procedure development.Arizona Care Network has proven to be an excellent environment for me as I enjoy a dynamic, rapidly evolving work environment, excel at identifying and creating improvement solutions, thrive on change and actively seek new opportunities for learning and growth.
