Family Resource Specialist - Connections To Work Program

Serve as Case Manager for Family Stabilization Services MFIP participants, who have significant physical, mental, or social barriers to employment. Establish and maintain long-term relationships with participants through proactive and reliable service. Utilize motivational interviewing and other coaching techniques that emphasize participant strengths and reinforce change behaviors. Assist participants in developing goal plans for family stability and self-sufficiency. Implement MFIP rules/regulations in order to encourage client engagement. Perform comprehensive assessments with participants to determine family strengths and needs and ensure ongoing program eligibility. Assist participants in accessing a wide array of services and resources, including affordable housing, employment and training programs, legal and financial assistance, health services (physical, mental, chemical), child enrichment programs and family support services.Coordinate services with partnered services providers; track participant activities and follow-through to monitor progress. Maintain accurate and precise case notes and file documentation, in both paper files and online databases.
