Co-ordinated complex and ongoing support arrangements for 23 individuals with disabilities and their families. Facilitated and organised, the employment of suitable support staff that were approved by the supported individual and the family.Formally reviewed the vision and support arrangements of a supported individual every six months, in addition to daily phone/e-mail/text correspondence. Advocate on behalf of the individual and family for access to mainstream or alternative services. Also, encouraged self-advocacy for individual’s building their capacity to navigate access to the NDIS and additional service providers.Attended professional development workshops relating to case work management, supports co-ordination, theories and practices for Social Work in the NDIS.Assisted individuals and families with consultation, template resources and drafted pre-planning materials for the NDIS rollout for Brisbane in July 2018.Involved in Mamre community events outside of standard work hours.Provided consultation and advice for the scoping of the Pro-Sims program, to identify and improve efficacy for recruiting and rostering support staff.
