- Provide Care Coordination/Management functions for adult members that are in need of carecoordination or need assistance to transition between services.- This position is tasked with ensuring that members from this specific group have access toservices, receive continuity of care, promote families and natural supports, improve functionaloutcomes, and maximize the efficient and effective use of resources.- Conduct screenings and comprehensive assessments to ensure whole-person care is provided.- Conduct community and/or in-home meetings with members based on member preference.- Develop care plans and assist members in achieving goals.
