Knowledge of the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing Service Coordination, drug and alcohol use and abuse by the elderly, and mental health issues. This include evaluation of health, psychological and social needs, development of an individually tailored case plan for services, and periodic reassessment of the residents' situation and needs. Social Service Coordinators can also set up a Professional Assessment Committee (PAC) to assist in performing initial resident assessments. Education residents on service availability, application procedures, clients' right, etc. Coordinate on-site programs, such as Food Box programs, Visiting Community Nurses/Wellness Programs, Senior Companions, podiatry, etc. Establish linkages with agencies and service providers. Monitor the ongoing provision of services from community agencies and keep the provider agencies current with the progress of the individual. Demonstrate resident confidentiality and code of ethics, and comply with Fair Housing Laws. Work and consult with tenant organizations and resident management team. Provide training to the project's residents in the obligations of tenancy or coordinate such training. Create and publish a monthly Newsletter for residents. Submit required reports in designated formats. These include the HUD-92456 Semi- Annual Performance Report for the Multifamily Housing Service Coordinator Program, Monthly Service Coordinator Reports, etc. Provide counseling or mediation as necessary.

Manage retirement and nonretirement mutual fund accounts for Vanguard clients. Main duties includes processing incoming checks into clients’ various accounts, calculating accounts’ yearly nontaxable limits, manage existing accounts such as change of address, adding international banks’ wiring option, etc., as well as out calling clients to clarify instructions.
