Alameda County Care Alliance - Advanced Illness Care Program• Provide front-line support for members of the community, health care systems, and ACCA congregations.• Implement person-centered and holistic approach to relationship building, service delivery and case management while prioritizing five cornerstones of spirituality, health, social needs, caregiving and advance care planning.• Deliver services in three stages through a series of 5-12 visits over a six-month period.• Utilize foundational skills in motivational interviewing, storytelling and story-listening to better understand multiple needs of each participant.

Tracing Health Program• Contribute to a network of contact tracers throughout the West Coast to track the spread of COVID-19.• Communicate with cases (people newly diagnosed with COVID-19) and contacts (people who have been in close contact with cases) in a professional and empathetic manner.• Call cases to provide support, gather information, discuss potential exposure, provide education, recall close contacts, and clarify isolation and quarantine procedures.• Collect and record information on symptoms into a Client Resource Management platform (CRM); trained in Microsoft Dynamics (CREST and MARI) and Salesforce (CalConnect). • Work independently, yet as part of a team to ensure that all cases and contacts are reached in a timely manner.
