I currently work in a family practice residency clinic. I work with the providers and their patients in managing their healthcare needs. I provide transitional care management (TCM) for patients being discharged from the hospital. I follow-up with our sickest patients on a monthly basis as part of the Medicare chronic care management (CCM) program. I triage both in clinic and over the phone as well as meeting with our patients for education purposes such as nutrition or diabetes management.

I spent the first 3 years of my nursing career working with women & children in the birth center at Valley Medical Center. I began as a labor & delivery nurse, working with antepartum, laboring, and postpartum women and their newborn babies. I also helped develop the perinatal loss committee to help develop policies and procedures to better serve our families experiencing the loss of a pregnancy or newborn. I then transitioned to a maternal-child float pool position where I began training to take care of low-acuity NICU patients in addition to postpartum couplets.
