As Facility Kaiser Liaison--in collaboration with doctors, patient care coordinator/case manager, discharge planner, rehab director, and interdisciplinary team--implemented a seamless transition of patients (and family) from the day of admission until after discharge. In the duration of patient stays: In-depth review of hospital orders, accuracy of data entry, proper documentation, patient assessment, patient education, medication reconciliation, patient representative as the Kaiser affiliate, direct communication with MD, proper communication to patient and family, constant communication with all disciplines (nursing, pharmacy, labs/diagnostics, specialty clinics, rehabilitation/therapy, dietary/nutrition, housekeeping), proper task delegation and accurate reporting to nurses, appointment scheduling, ensuring follow-ups are done, transportation set-up and assistance on discharge plans whether to home, ALF/B&C, custodial care, palliative, or hospice.
